It takes a whole lot to annoy me. I have a high threshold... but I guess the only way you can, is to be rude, icy, or an asshole. I will easily get put out by that.
Preach outside a proper forum in front of me and make no effort to consider an opposing viewpoint. I guarantee that I will rip you a new asshole, piss you off and make you look like an idiot for not knowing what the fuck you're talking about. And then I'll guffaw when you go away mad. You want to convert me? You want to save my soul? You better know your shit and have all your chinks repaired because if I find just one flaw, I'm going to make you cry and relish doing so. I eat zealotry like a pot-head eats torilla chips. On this matter, my vitriolic rightiousness knows no bounds.

Oh God, a lot of things. Primarily, people using the excuse "it's just the way I am, I tell it like it is" to be rampantly rude and obnoxious. Also people letting their kids/dogs run riot on the streets, racists like the idiot who drove me home in a taxi yesterday, orange clothing, Lee Evans, people I love not keeping their promises and burning my toast.
Doing anyhitng which is selfish, ugly, hurtfull or dangerous to a disenfranchized person or creature. I can tolerate a lot of shit, but not seeing people be mean spirited toward the disabled, elderly, ethnic minorities, poor, etc. I could see myself going postal over this issue.
And, more personally, telling me what I feel. It boils my blood in no time flat.
Lie to me when I know the truth. Alternately, indecision really gets my goat to no end. And four of my closest friends are the most indecisive people I've ever met in the world.
There are oh so many ways. Yelling "Mom, mom, mom, mom, mom, mom, mom, mom, mom, mom, mooooooooooooooahahahahaom!". Lieing. Asking something stupid when I know you know better. Stealing from me. Tapping my shoulder. Ignoreing me. Telling me I am wrong when I can prove otherwise. Leaveing a mess. Expecting me to cover your ass for you. Discussing my money without my permission. Insurance companies. Whineing. Not showering. I could go on and on and on!
Except at 7-11. Ever since they implemented the self-serve slurpee machines and chili nacho bar you don't have to try to learn the history, philosphies nor the languages of the various people you find there behind the counter at 3am after working a late night at work. There are somethings you just never want to hear from some people alone in an isolated convience store.
